Appointments.

Patients are seen by appointment only. If you are unable to keep your appointment, please contact the office 24 hours in advance to reschedule. This will allow us to accommodate another patient that may need to be seen.

Billing.

As a courtesy to you, we will file your insurance for any office and/or hospital services. Payment is expected at the time of your visit. You will be asked to pay your co-pay and/or deductible at the time of service if you have insurance coverage.

Our clinic participates in several preferred provider organizations. Please contact your insurance company to verify if we are in your network.

Please bring your insurance cards to each visit. We ask that you let us know if your insurance coverage has changed since the last time you were in the clinic.

As your provider we have a professional relationship with you. Ultimately you are responsible for your bill. We ask that you work directly with your insurance carrier in the event they do not pay accordingly.
